Thank you for choosing counselling. Please read the information below before signing.

• Confidentiality - All sessions are confidential. Information may only be shared if there is risk of harm to yourself or others, or disclosure is required by law.

• Session Notes & Records -  Brief notes are kept securely for professional care and supervision. Records remain confidential and are not shared without your consent.

• Supervision - To maintain quality of care, I may discuss my work with a professional supervisor. Your identity is protected.

• Your Rights - You may ask questions at any time. You can stop counselling whenever you choose. You may request access to your records.

• Professional Standards - I am a registered member of the New Zealand Association of Counsellors (NZAC) and follow its Code of Ethics.

• Recordings - Recording of sessions is not permitted.

• Fees & Cancellations - For paying clients, fees are payable before each session. Cancellations with less than 24 hours’ notice will be charged in full.

• Emergencies Counselling is not a crisis service. For urgent help, please call 1737, Lifeline, or your GP.

• Online Sessions - For online/phone sessions, please ensure you are in a private space. Be aware of potential technology risks.

• Ending Counselling - Either you or I may end counselling at any time. If this happens, I will discuss referral options with you.

• Complaints - Concerns may be raised directly with me, or through the NZAC complaints process.

• EAP Sessions - If sessions are funded by your workplace, these sessions may be limited. We will agree on priorities within that time frame.
